Food Allergy Plan - to be submitted for student who have moderate/severe food allergic reaction, accompanied with Medical Consent Form

Seizure Action Plan - to be submitted for student who have a diagnosis of Seizure, accompanied with Medical Consent Form

Medication Order Form - to be submitted for student who needs medication in school for learning or life-threatening condition, accompanied with Medical Consent Form.

Medical Consent Form - to be accompanied with any plan and order, to share information with school staff, consent for screening, consent for physical done on school ground.

Universal Child Record - this is a sample physical form to be filled out by student's primary doctor

Immunization Exemption Form - to be submitted as an alternative option by parent/guardian who are choosing to exercise religious exempt rights regarding immunizations.
